It starts as a small red bump. In a day or two it becomes hot, swollen and throbbing, and sitting, walking or sleeping becomes hard. Many families in Toba Tek Singh, Gojra, Kamalia, Pir Mahal and Shorkot call it a phora and try the usual things first: a hot compress, a home paste, a needle, or squeezing it. Some small boils do settle on their own. Others turn into an abscess, a pocket of pus under the skin, that needs a surgeon.

This post explains when a boil or abscess needs a surgeon’s examination, and when it needs same-day care. It is education only. It is not an online diagnosis, and no outcome is guaranteed. Only an in-person examination can tell what is going on.

Boil or abscess: what is the difference?
A boil is usually an infected hair root. It is small and often comes to a head. An abscess is a deeper, larger collection of pus. It is painful, hot and tense, and it can keep growing. Antibiotics alone often do not clear an abscess, because the medicine cannot reach the pus well. Many abscesses need to be opened and drained properly by a surgeon, in clean conditions and with pain relief.

Signs that you should see a surgeon
– The swelling is bigger than a large coin, or keeps getting bigger
– It is still painful after two or three days of warm compresses
– You have fever, shivering, or feel generally unwell
– The redness is spreading around the swelling
– It keeps coming back in the same place, or new ones keep appearing
– It is on the face, near the eye, on the breast, in the armpit or groin, or near the back passage
– You have diabetes, kidney problems, or take steroids or medicines that lower immunity

Why you should not squeeze or cut it at home
Squeezing can push infection deeper and spread it. A needle or blade at home, without cleaning and pain relief, often leaves pus behind and the abscess fills again. Random pastes and powders can irritate the skin and hide how bad the infection is. If it needs opening, let a surgeon do it properly.

Boils that keep coming back
Repeated boils in the same place can have a reason behind them. Uncontrolled blood sugar is common in our area, and sometimes a boil is the first sign of diabetes. A boil that keeps coming back near the tailbone can be a pilonidal sinus. One that keeps coming back beside the back passage can be a perianal abscess or a fistula. Repeated boils in the armpit or groin can come from a long-term skin condition. These problems tend to return unless the cause is found and treated. An examination decides what care fits you, including laser options for selected perianal and pilonidal cases.

An abscess near the back passage needs special attention
Severe pain beside the anus, with swelling, fever, or pain when sitting, should be checked soon. Some of these abscesses lead to a fistula if they are not treated properly. Please do not feel shy. A surgeon sees these problems every week, and the examination is private and respectful.

What to note before your visit
– When it started, and whether it has grown or burst
– Any fever, and how many times it has come back before
– Whether you have diabetes, and your latest sugar report if you have one
– Medicines and antibiotics you have already taken

Red-flag signs: get same-day care, do not wait for the next OPD
Seek urgent hospital or surgical assessment the same day if you notice:
– High fever, shivering, confusion, or feeling faint
– Redness or swelling spreading fast, or skin turning dark or very painful beyond the swelling
– A swelling on the face with pain around the eye, or swelling that makes it hard to swallow or breathe
– A very painful swelling near the back passage with fever or difficulty passing urine
– Any abscess in a person with diabetes who feels unwell or has very high sugar

How Habib Memorial OPD helps
Care starts with listening and a calm, respectful examination. The surgeon checks the swelling, explains what it is likely to be, and advises whether a sugar test, blood test or ultrasound is useful. Options may include dressings and medicines, a small drainage procedure when it is truly needed, or a plan to treat the cause of boils that keep coming back. Every decision is made after examination and informed consent, never from a blog or an advertisement. If you live in Toba Tek Singh or nearby towns, Habib Memorial is the main place for assessment.
