Many people in Toba Tek Singh, Gojra, Kamalia, Pir Mahal and Shorkot notice a small opening near the anus that keeps leaking pus or fluid. Underwear may stay soiled. Pain or swelling may come and go. Sometimes this starts after a boil or abscess near the anus that drained once, then never fully settled. This pattern often points to an anal fistula (fistula-in-ano) — a small tunnel between the inside of the anal canal and the skin nearby.
This post explains, in plain words, what an anal fistula is, which signs to notice, which warning signs need same-day care, and how laser treatment (and other options) are usually discussed only after examination. It is education only. It is not an online diagnosis, and no outcome is guaranteed. Your own plan must come from an in-person examination.
What is an anal fistula?
An anal fistula is a narrow tunnel that connects the inside of the anal canal to a small opening on the skin near the anus. It often follows an abscess (a collection of pus) that has drained. When the infection finds a path to the skin but the tract does not heal fully, a fistula can remain. Liquid, pus or soiling may keep coming from that outer opening.
Signs people commonly notice
– Recurrent pus or watery discharge from a small hole near the anus
– Soiling of underwear even when bowels feel fine
– Pain, swelling or a tender lump that comes and goes near the anus
– A small opening you can see or feel in the skin beside the anus
– Discomfort when sitting for long, or after long journeys
– A past boil or abscess near the anus that drained but never fully healed
Piles (haemorrhoids), fissure (a tear), and fistula are different problems. Discharge from a small opening, especially after an abscess, is more typical of fistula than of piles alone. Only an examination can confirm what you have.
Why should it be checked?
A fistula does not usually close on its own. Leaving it can mean repeated infections, more discharge, and sometimes a more complex tract over time. An early, calm OPD examination lets the surgeon confirm whether a fistula is present, assess its path as far as clinical examination allows, and explain options for your situation — including when laser may be discussed — while there is time to plan.
Red-flag signs: get same-day care, do not wait for the next OPD
Seek hospital care the same day if you have:
– Fever with increasing pain near the anus
– Spreading redness, warmth or swelling of the buttock or perineum
– Severe pain that makes sitting or walking very difficult
– Sudden hard swelling that feels like another abscess
– Feeling very unwell, shivering, or rapidly worsening discharge with pain
These can mean an active infection that needs urgent assessment, not a wait for the next routine appointment.
How laser treatment is usually discussed (after examination)
Laser is one option that may be discussed for selected fistulas after a proper examination — not a guarantee, and not suitable for every case. The surgeon first confirms the diagnosis, looks at the outer opening, and assesses the tract clinically. Imaging may be advised in some cases. Depending on the type and path of the fistula, the discussion may include laser-assisted methods, seton (a soft thread that helps drainage and staged care), or other surgical approaches. Benefits, risks, expected recovery and the need for follow-up are explained only after examination. Nothing is done without your informed consent. Online videos cannot decide which method fits you.
Common misunderstandings
– “Ointments or antibiotics alone will close the tunnel.” Medicines may calm infection for a while, but they do not usually close a established fistula tract.
– “Laser cures every fistula in one go.” Laser is one tool among several. Suitability depends on your own fistula after examination; outcomes are never guaranteed.
– “If the discharge stops for a few days, it is cured.” Fistulas often quieten then return. A quiet spell does not mean the tract has healed.
– “I can self-diagnose from photos online.” Piles, fissure, pilonidal problems and fistula can look confusing from home. Only an in-person exam is safe.
Before your OPD visit
Note when the discharge started, whether you had a boil or abscess before, how often underwear soiled, and any fever or swelling. Bring a list of your medicines and any old reports or ultrasound/MRI if you have them. Wear loose clothing so examination is easier. Do not start strong painkillers that hide fever without advice if you feel unwell.
How Habib Memorial OPD helps
Care starts with listening and a calm examination, done with privacy and respect. The surgeon checks the area, looks for an outer opening, and explains whether watchful review, further tests, or a treatment discussion (including laser when appropriate) fits you. Every decision is made after examination, never from a blog or an advertisement. If you live in Toba Tek Singh or nearby towns, Habib Memorial is the main place for this assessment.
