Sometimes a person in Toba Tek Singh, Gojra, Kamalia, Pir Mahal or Shorkot wakes up, or comes back from a long journey, with a sudden, painful lump at the edge of the anus. It may look bluish or dark, feel hard like a small grape, and make sitting and walking very uncomfortable. Very often this is a thrombosed external pile: a small blood clot that has formed inside a pile just under the skin.

This post explains, in plain words, what a clotted pile is, why it happens, which signs need same-day care, and how a surgeon usually decides what to do after examination. It is education only. It is not an online diagnosis, and no outcome is guaranteed. Your own plan must come from an in-person examination.

What is a thrombosed (clotted) pile?
Piles (haemorrhoids, bawasir) are cushions of blood vessels around the anus. External piles sit under the skin at the anal edge. When blood inside one of these veins suddenly clots, the area swells quickly and becomes tense and painful. That clot is what people feel as a hard, tender lump.

Why does it happen?
Common triggers include:
– Hard stool, constipation and straining in the toilet
– Bouts of diarrhoea
– Long sitting, including long bus or car journeys
– Heavy lifting or strenuous exercise
– Pregnancy and the days after childbirth
– Sometimes there is no clear reason

Signs people commonly notice
– A sudden, hard, tender lump at the edge of the anus
– Bluish, purple or dark colour of the lump
– Pain that is worst in the first few days, especially when sitting or passing stool
– Swelling around the anal opening
– Sometimes a little bleeding if the skin over the clot breaks

A clotted pile is different from bleeding internal piles, a fissure (a small tear that hurts sharply during stool), or an abscess (a collection of pus, often with fever). These can feel similar at home. Only an examination can tell them apart safely.

Red-flag signs: get same-day care, do not wait for the next OPD
Seek hospital care the same day if you have:
– Fever, shivering, or spreading redness and heat around the anus or buttock
– Pus, or a swelling that keeps getting bigger and more painful
– Heavy bleeding, passing clots, or feeling faint or dizzy
– Piles from inside that have come out, turned dark, and cannot be pushed back
– Severe pain with difficulty passing urine
– Bleeding while you are on blood-thinning medicines

These can mean infection, a different problem, or a complication that needs urgent assessment.

Please do not do this at home
– Do not squeeze, prick or try to burst the lump. This can cause infection and heavy bleeding.
– Do not apply strong pastes, heated oils or home “cutting” remedies to the area.
– Do not hide fever with strong painkillers and wait. If you feel unwell, get checked.

How a surgeon usually decides (after examination)
The surgeon first confirms that it really is a clotted external pile and not an abscess, fissure or another problem. The decision then depends on how long the lump has been there, how painful it is, and your general health. Broadly:
– For some patients seen early, while pain is at its worst, a small procedure under local anaesthesia to remove the clot may be discussed.
– For many others, especially when pain is already settling, care focuses on comfort and soft stools while the clot is absorbed by the body over time.
– If piles keep coming back, longer-term options, including laser treatment for selected piles, may be discussed at a later, planned visit.

Benefits, risks and expected recovery are explained only after examination. Nothing is done without your informed consent, and no single method suits everyone.

Simple habits that help prevent piles
– Drink enough water through the day, especially in hot weather
– Eat fibre: vegetables, fruit, whole-wheat roti, daal, and ispaghol if your doctor agrees
– Go to the toilet when you feel the urge, and do not sit there for long or use the phone
– Avoid straining; on long journeys, take short breaks to stand and walk

Before your OPD visit
Note when the lump appeared, what may have triggered it, how the pain has changed, any bleeding, fever or urine trouble, and a list of your medicines (especially blood thinners). Wear loose clothing so examination is easier and more comfortable.

How Habib Memorial OPD helps
Care starts with listening and a calm, private examination, done with respect.
