Many people in Toba Tek Singh, Gojra, Kamalia, Pir Mahal and Shorkot get an ultrasound for something else — stomach upset, a check-up, pregnancy planning, or liver tests — and the report suddenly says cholelithiasis or gallstones. There was no severe pain. No fever. No yellow eyes. The stones were found by chance. Doctors often call these silent or asymptomatic gallstones.

This post explains, in plain words, what silent gallstones on ultrasound usually mean, when watchful waiting is reasonable, when a surgery discussion is useful, how symptoms can start later, and which red-flag signs need same-day care. It is education only. It is not an online diagnosis, and no outcome is guaranteed. Your own plan must come from an in-person examination and your reports.

What does “silent gallstones” mean?
The gallbladder is a small bag under the liver. It stores bile, a juice that helps digest fat. Stones can form inside it. Silent means the stones are there on the scan, but you have not had the typical gallbladder pain attacks (often upper-right or upper-middle belly pain after a fatty meal, with nausea). Many people live with quiet stones for years and never need an operation. Others stay quiet for a while and later develop symptoms. A report alone does not decide the next step — history and examination do.

Why were they found on ultrasound?
Ultrasound is a common, safe scan for the upper belly. It often picks up stones even when the reason for the scan was something else. The report may also mention sludge, wall thickness, or whether the bile duct looks dilated. Bring the full report (and any older scans) to OPD so the surgeon can read what was actually written, not only what a neighbour summarised.

Watchful waiting — when it may be reasonable
For many people with truly silent stones, a calm plan is to:
– Understand the report in person
– Know the warning symptoms to watch for
– Keep a healthy, home-cooked eating pattern without extreme fad diets
– Return for review if new pain, fever, vomiting, or yellowing of the eyes starts

Watchful waiting is not the same as “ignore forever.” It means you and your doctor have agreed that, for now, monitoring and education are enough. That decision belongs in clinic, after your history and examination.

When a surgery discussion is useful even if pain is mild or absent
An operation is not automatic for every silent stone. A surgeon may still discuss gallbladder removal (cholecystectomy) after examining you when, for example:
– You have had even one clear biliary-type pain attack that you had forgotten or blamed on “gas”
– Stones are large, or there are many, and your doctor is concerned about future complications in your situation
– There are concerning findings on the scan (such as a thickened wall, suspected polyp with worrying features, or other notes that need specialist review)
– You have another health reason that makes a planned discussion wiser than waiting for an emergency
– You are planning major travel or weight-change programmes and want a clear plan before symptoms hit

These are discussion points, not a checklist that forces surgery. Benefits, risks, and timing are explained only after examination and informed consent. No blog can decide for you.

How symptoms often start later
Silent today can become symptomatic later. Common patterns families describe include:
– Upper-right or upper-middle belly pain after oily or heavy meals
– Pain that comes in waves, lasts hours, and may go to the right shoulder or back
– Nausea or vomiting with the pain
– Attacks that settle, then return days or weeks later

If this pattern begins, do not rely on painkillers alone as a “cure.” Book a proper surgical review so the cause can be checked.

Red-flag signs: get same-day care, do not wait for the next OPD
Seek urgent hospital assessment the same day if you notice:
– Severe ongoing upper-belly pain that does not settle
– Fever with belly pain
– Yellow eyes or yellow skin (jaundice), dark urine, or pale stools
– Persistent vomiting and inability to keep fluids down
– Confusion, severe weakness, or a very unwell appearance with gallbladder symptoms

These can signal infection, blocked bile flow, or related problems that need same-day assessment — not a wait for the next routine slot.

Common misunderstandings about silent stones
– “No pain means the stones will melt with vinegar or herbal paste.” Pastes and home remedies do not reliably dissolve stones, and they can delay the right review.
– “If the ultrasound found stones, I must have surgery this week.” Many silent stones are observed; surgery is a shared decision after examination.
– “I can ignore the report forever because I feel fine.” Feeling fine today is good news — still learn the red flags and keep the report for future visits.
– “Cutting all oil forever will remove the stones.” Reducing very heavy fried food may reduce triggers if you become symptomatic; it does not guarantee stones will disappear.

How Habib Memorial OPD helps
Care starts with listening and a calm examination. The surgeon reviews your ultrasound and any blood tests, asks about past “gas” attacks that might actually have been biliary pain, and explains whether watchful waiting, further tests, or a surgery discussion fits your situation. Every decision is made after examination, never from a blog or an advertisement. If you live in Toba Tek Singh or nearby towns, Habib Memorial is the main place for this assessment.
