“Doctor sahib, there is pain and a small hole near my tailbone. Sometimes it swells and drains pus, then settles — then it comes back.” We hear this often from young adults in Toba Tek Singh, Gojra, Kamalia, Pir Mahal, and Shorkot — especially people who sit long hours (drivers, office work, students) or notice more hair and sweat in that fold.

This can be a pilonidal sinus (also called a pilonidal cyst or abscess when infected). It sits at the top of the buttock cleft, near the tailbone — not at the anus itself. This post explains, in simple words, what it is, who gets it, why it returns after simple drainage alone, when to see a surgeon, and how laser or other modern options may be discussed after a proper exam. It is education only. It is not an online diagnosis, and no outcome is guaranteed. Only examination by a surgeon can tell what your swelling or discharge is.

What is a pilonidal sinus?
A pilonidal sinus is a small tract or pocket under the skin in the cleft between the buttocks, near the top (tailbone area). Hair, debris, and dead skin can collect inside. When bacteria grow, it becomes an abscess — painful, swollen, and often draining pus or blood-stained fluid. Between flare-ups some people only feel a tender spot, a dimple, or a tiny opening.

It is different from piles, fissure, or fistula near the anus. Location matters: pilonidal problems sit higher up in the natal cleft, not at the anal opening.

Who is more at risk?
It is more common in:
– Young adults (teens and twenties are typical, but it can appear later)
– Men more often than women, though women get it too
– People who sit for long periods — drivers, desk jobs, long travel
– Thick or coarse body hair in the cleft, with sweating and friction
– Overweight, or clothing that rubs the area for hours
– A family history of similar problems in some cases
– Past incomplete drainage of an abscess in the same spot

Common signs
– Pain, swelling, or a lump near the top of the buttock cleft / tailbone
– Redness and tenderness that make sitting or driving uncomfortable
– Discharge of pus or blood-stained fluid from a small hole (sinus opening)
– Recurrent abscesses in the same place after “drainage and antibiotics”
– Sometimes fever with a large, hot swelling

Why does it come back after drainage alone?
Draining an abscess relieves pain and infection in the short term. That is important when there is a collection of pus. But drainage alone often leaves the underlying tract and hair-filled pocket in place. When hair and debris keep collecting, infection can return weeks or months later. That is why many people need a planned assessment for a definitive approach once the acute infection settles — not only repeated emergency drainage.

When laser or modern options may be discussed
After examination (and sometimes a simple scan if needed), the surgeon explains options suited to your findings. Depending on how many tracts there are, whether infection is active, and your overall health, possibilities may include careful wound care after drainage, excision of the sinus tract, flap or off-midline techniques in selected cases, or laser-assisted methods when the anatomy and infection status make them suitable.

Laser is not a magic button and is not suitable for every pilonidal problem. It is one tool among several. Any procedure is planned only after findings, a clear talk about benefits, risks, recovery, and after-care, and your consent. No ethical surgeon guarantees “no pain, no recurrence, overnight cure.” Prefer an in-person exam over WhatsApp photos or online advice.

When to see a surgeon (planned OPD)
Book a calm Habib Memorial OPD visit if you notice:
– Recurrent swelling or discharge near the tailbone
– Pain that keeps interrupting sitting, driving, or study
– A small hole that keeps draining even when swelling goes down
– Past abscess drainage in the same area, with return of symptoms
– Uncertainty whether the problem is pilonidal or something near the anus

Emergency warning signs — seek same-day care
– Rapidly spreading redness, severe pain, or a large hot abscess
– Fever, chills, or feeling very unwell with the swelling
– Inability to sit or walk because of pain
– Spreading infection toward the back or thighs

Do not wait for the next OPD date for these. Acute drainage may be needed first; definitive planning often follows once infection is controlled.

Myths we hear often
– “It is just a boil — squeeze it and it will finish.” Squeezing can push infection deeper and delay proper care.
– “Antibiotics alone always cure it.” Antibiotics help surrounding infection; they do not remove the hair-filled tract.
– “If it drained once, it will never return.” Many pilonidal sinuses recur until the tract is properly addressed under clinical guidance.
– “Laser cures every case overnight with zero after-care.” Not true. Laser may help selected cases after exam and consent; after-care and hygiene still matter. Outcomes vary.
– “It is the same as piles or fistula.” Location and cause differ. Only examination clarifies the difference.

How Habib Memorial OPD helps
At a calm OPD visit, the surgeon listens to your history, examines the cleft carefully, and checks for openings, tracts, or active infection. If needed, further assessment is advised. Then options are explained in plain words — including when laser may or may not be suitable. Female patients can bring a family member. Any operation is planned only after findings, informed discussion, and consent.

Living abroad?
If you or a family member is in the Gulf or overseas, you can send existing reports or photos of discharge notes on WhatsApp first, and the doctor will guide you on the next step personally. No online diagnosis is given. If you are planning a visit home, plan time for assessment, any procedure, and follow-up before you fly back.
