“Doctor sahib, my C-section was years ago, but now there is a soft swelling at the scar that gets bigger when I cough.” We hear this often from patients in Toba Tek Singh, Gojra, Kamalia, Pir Mahal, and Shorkot. Many people think it is just loose skin or fat, or feel shy to show an old scar, and they wait for years.
A swelling at or near an old operation scar can be an incisional hernia. This post explains, in simple words, what it is, who is more at risk, the warning signs, and when to see a surgeon. It is education only. It is not an online diagnosis, and no outcome is guaranteed. Only an examination by a surgeon can tell what your swelling is.
What is an incisional hernia?
When the belly is opened for an operation, the muscle layer is stitched closed. Sometimes that repair becomes weak over time. Then fat or a loop of bowel can push through the weak spot and make a bulge under the skin. It can appear months or even years after the operation.
It can happen after many kinds of belly surgery, for example:
– Caesarean section (C-section) or open hysterectomy
– Appendix operation
– Gallbladder or other open or keyhole belly surgery
– A large “open” operation done in an emergency
Who is more at risk?
Anyone who has had belly surgery can develop it, but the risk is higher with:
– A wound infection after the first operation
– Being overweight, or putting on weight after surgery
– Diabetes or smoking
– Long-term cough, constipation with straining, or difficulty passing urine
– Lifting heavy weight too soon after surgery (farm work, labour, gym)
– More than one operation through the same scar, or repeated pregnancies
Common signs
– A soft bulge at or near the old scar
– The bulge gets bigger when you stand, cough, lift, or strain, and may go smaller when you lie down
– A dragging feeling, discomfort, or ache at the scar, especially by the end of the day
– Some people feel no pain at all, only the swelling
Will it go away on its own?
No. A hernia does not heal itself. Over time many incisional hernias slowly get bigger. A belt or binder may make some people more comfortable, but it does not repair the weak muscle. Treatment is decided after examination. Depending on the size, your health, and the scan findings, the surgeon may advise watchful follow-up or a planned repair, and will explain the type of operation that suits you.
Emergency warning signs — go to the hospital the same day
– The bulge becomes hard, very painful, or tender
– It will not go back in when you lie down, when it used to
– The skin over it turns red, purple, or dark
– Vomiting, swelling of the whole belly, or not passing gas or stool
– Fever with pain at the swelling
These can mean the bowel is trapped. Do not wait for the next OPD date, and do not press hard on the bulge to push it back.
Myths we hear often
– “It’s just loose skin after pregnancy.” Loose skin does not usually bulge out when you cough. A bulge that grows with coughing or lifting should be checked.
– “A tight belt will cure it.” A belt may give support, but it does not close the gap in the muscle.
– “If it doesn’t hurt, it doesn’t matter.” Painless hernias can still grow, and some can become trapped. A check-up helps you plan calmly instead of in an emergency.
– “Second operations always fail.” Not true. Your surgeon will explain the options, the benefits and risks, and what helps recovery in your case.
Lowering the risk after any belly operation
– Follow wound-care advice and come back if the wound looks red, swollen, or has pus
– Avoid heavy lifting until your surgeon says it is safe
– Treat constipation and a long cough, and stop smoking
– Keep a healthy weight, and keep diabetes controlled

یہ کئی قسم کے پیٹ کے آپریشن کے بعد ہو سکتا ہے، مثلاً:
– بڑا آپریشن (سی سیکشن) یا اوپن بچہ دانی کا آپریشن
– اپینڈکس کا آپریشن
– پتے یا پیٹ کا کوئی اور اوپن یا دوربین آپریشن
– ایمرجنسی میں ہونے والا بڑا “اوپن” آپریشن
کن لوگوں کو زیادہ خطرہ ہے؟
پیٹ کا آپریشن کروانے والے کسی کو بھی یہ ہو سکتا ہے، مگر خطرہ زیادہ ہوتا ہے اگر:
– پہلے آپریشن کے بعد زخم میں انفیکشن ہوا ہو
– وزن زیادہ ہو، یا آپریشن کے بعد وزن بڑھ گیا ہو
– شوگر ہو یا سگریٹ نوشی کرتے ہوں
– پرانی کھانسی، قبض میں زور لگانا، یا پیشاب میں دقت
– آپریشن کے بعد بہت جلد بھاری وزن اٹھانا (کھیتی باڑی، مزدوری، جم)
– ایک ہی نشان سے ایک سے زیادہ آپریشن، یا بار بار حمل
عام علامات
– پرانے نشان پر یا اس کے قریب نرم ابھار
– کھڑے ہونے، کھانسنے، وزن اٹھانے، یا زور لگانے پر ابھار بڑا ہو جائے، اور لیٹنے پر چھوٹا ہو جائے
– نشان پر کھنچاؤ، بے آرامی، یا درد، خاص طور پر دن کے آخر میں
– کچھ لوگوں کو بالکل درد نہیں ہوتا، صرف سوجن ہوتی ہے
کیا یہ خود ٹھیک ہو جائے گا؟
نہیں۔ ہرنیا خود ٹھیک نہیں ہوتا۔ وقت کے ساتھ بہت سے انسیژنل ہرنیا آہستہ آہستہ بڑے ہوتے جاتے ہیں۔ بیلٹ یا پٹی سے کچھ لوگوں کو آرام ملتا ہے، مگر یہ کمزور پٹھے کو ٹھیک نہیں کرتی۔ علاج معائنے کے بعد طے ہوتا ہے۔ سائز، آپ کی صحت، اور اسکین کے نتائج کے مطابق سرجن نگرانی کے ساتھ فالو اپ یا منصوبہ بند آپریشن کا مشورہ دے سکتے ہیں، اور بتائیں گے کہ آپ کے لیے کون سا آپریشن موزوں ہے۔
ایمرجنسی علامات — اسی دن ہسپتال جائیں
– ابھار سخت، بہت دردناک، یا چھونے پر درد والا ہو جائے
– لیٹنے پر اندر نہ جائے، جبکہ پہلے چلا جاتا تھا
– اس کے اوپر جلد سرخ، جامنی، یا گہری ہو جائے
– الٹی، پورے پیٹ کا پھولنا، یا ہوا یا پاخانہ نہ نکلنا
– سوجن پر درد کے ساتھ بخار
یہ آنت کے پھنسنے کی علامت ہو سکتی ہیں۔ اگلی OPD تاریخ کا انتظار نہ کریں، اور ابھار کو زور سے دبا کر اندر کرنے کی کوشش نہ کریں۔
عام غلط فہمیاں
– “یہ تو حمل کے بعد کی ڈھیلی جلد ہے۔” ڈھیلی جلد عموماً کھانسنے پر باہر نہیں ابھرتی۔ جو ابھار کھانسنے یا وزن اٹھانے پر بڑھے اس کا معائنہ ضروری ہے۔
– “سخت بیلٹ سے ٹھیک ہو جائے گا۔” بیلٹ سہارا دے سکتی ہے، مگر پٹھے کا خلا بند نہیں کرتی۔
– “درد نہیں تو کوئی بات نہیں۔” بغیر درد والا ہرنیا بھی بڑھ سکتا ہے، اور کچھ پھنس بھی سکتے ہیں۔ بروقت معائنہ ایمرجنسی کے بجائے سکون سے منصوبہ بندی میں مدد دیتا ہے۔
– “دوسرا آپریشن ہمیشہ ناکام ہوتا ہے۔” یہ درست نہیں۔ آپ کے سرجن آپشنز، فوائد اور خطرات، اور آپ کے کیس میں صحت یابی میں مددگار باتیں سمجھائیں گے۔
پیٹ کے کسی بھی آپریشن کے بعد خطرہ کم کرنے کے طریقے
– زخم کی دیکھ بھال کی ہدایات پر عمل کریں، اور زخم سرخ، سوجا ہوا، یا پیپ والا لگے تو واپس آئیں
– جب تک سرجن اجازت نہ دیں بھاری وزن نہ اٹھائیں
– قبض اور پرانی کھانسی کا علاج کروائیں، اور سگریٹ چھوڑ دیں
– وزن مناسب رکھیں، اور شوگر کنٹرول میں رکھیں
