Bleeding, pain, itching, or discharge around the anus can be worrying. But these symptoms do not always mean the same problem. “Piles,” fissures, and fistulas are different conditions and may need different treatment.
This simple guide can help you understand the usual pattern. It cannot diagnose you. Only a proper examination can confirm the cause—not a WhatsApp photo.
The quick comparison
| Condition | What it is | Common pattern | Bleeding, pain, or discharge? |
|---|---|---|---|
| Piles (haemorrhoids) | Swollen veins in or around the anus | Bright-red blood during or after passing stool; itching or a lump may occur | Bleeding is common. Pain may occur with an external clot or a prolapsed pile. Discharge is not usually the main symptom. |
| Anal fissure | A small tear in the lining of the anus | Sharp, cutting or burning pain during and after stool, often after constipation or a hard stool | Pain is usually the main symptom. A small amount of bright-red blood may appear on the tissue or stool. Discharge is not typical. |
| Anal fistula | An abnormal tunnel between the inside of the anus/rectum and the skin nearby | Repeated swelling, a sore spot, or a small opening that may drain and then settle temporarily | Discharge or pus is more suggestive. Pain and swelling can flare, especially if drainage stops. Bleeding is not usually the main symptom. |
These patterns can overlap. For example, a person may have pain and bleeding with more than one condition. A lump, blood, or discharge does not prove which condition is present.
Piles: bleeding is often the clue
Piles are swollen blood vessels in the anal canal or under the skin around the anus. Internal piles commonly cause painless, bright-red bleeding with stool. Itching, mucus, a feeling of incomplete emptying, or tissue coming out may also occur.
An external pile can become painful, especially if a clot forms. Do not assume every episode of rectal bleeding is piles; other causes also need consideration.
Fissure: pain is often the clue
An anal fissure is a small tear in the anal lining. It may happen after passing a hard stool, constipation, or repeated diarrhoea. The pain can feel sharp or like a cut, and it may continue for minutes or hours after a bowel movement. A little bright-red blood is also possible.
Keeping stools soft and avoiding straining may help a new, mild fissure settle. Ongoing or recurrent pain needs a clinical assessment because some fissures require specific treatment.
Fistula: discharge is often the clue
An anal fistula is a small abnormal passage connecting the inside of the anus or rectum to nearby skin. It can develop after an infection or an abscess. Typical clues include repeated swelling in one spot, pus or watery discharge, a bad smell, or an opening near the anus. Pain may improve briefly when fluid drains, but the problem can return.
A fistula usually does not resolve permanently with home care alone. A surgeon should assess it and discuss the appropriate treatment.
What can you do briefly at home?
For mild symptoms, while arranging medical advice, you may try:
- Drink enough water unless a clinician has told you to restrict fluids.
- Increase fibre gradually through foods such as fruit, vegetables, whole grains, and pulses.
- Avoid straining and do not sit on the toilet for a long time.
- A comfortably warm sitz bath may soothe discomfort.
- Keep the area gently clean and dry; avoid rubbing.
These steps may ease symptoms but do not confirm the diagnosis or cure every cause. Avoid using steroid, numbing, antibiotic, or other rectal creams for a long time without medical advice.
When should you see a surgeon?
Arrange an examination if bleeding returns, pain is significant or persistent, a lump keeps coming back, there is pus or ongoing discharge, or symptoms are not improving. You should also be assessed if bowel habits change or if you are unsure whether the blood is from piles.
Seek urgent medical help for heavy or continuous bleeding, dizziness or fainting, fever, rapidly worsening swelling, severe constant pain, or difficulty passing stool or urine.
Myth vs fact
Myth: “Any bleeding is just piles.”
Fact: Piles can bleed, but bleeding has several possible causes. Recurrent or unexplained bleeding deserves an examination.
Myth: “A WhatsApp photo can confirm a fissure or fistula.”
Fact: A photo cannot reliably show the full problem. Only an in-person examination, and sometimes further tests, can confirm the diagnosis.
Myth: “If the discharge stops, the fistula is cured.”
Fact: A fistula may drain and settle temporarily, then return. A surgeon’s assessment is important.
Myth: “Pain always means piles.”
Fact: Sharp pain with stool is often seen with a fissure; severe swelling or pain can also occur with other problems.
The key message
Bleeding points more often towards piles, sharp pain with stool towards a fissure, and repeated discharge towards a fistula—but these are only clues. Do not self-diagnose. An examination is the safest way to identify the cause and choose the right treatment.

Maqad ke ird-gird khoon, dard, khujli ya risaao pareshan kar sakta hai. Lekin in alamat ka matlab hamesha ek jaisa nahi hota. Bawaseer (piles), fissure aur fistula teen mukhtalif masail hain aur in ka ilaaj bhi mukhtalif ho sakta hai.
Yeh seedhi si maloomat aap ko aam farq samajhne mein madad de sakti hai. Yeh online tashkhees nahi hai. Asal sabab sirf munasib muaina se maloom hota hai—WhatsApp photo se nahi.
Mukhtasar muqabla
| Masla | Kya hota hai? | Aam alamat | Khoon, dard ya risaao? |
|---|---|---|---|
| Bawaseer (piles) | Maqad ke andar ya ird-gird ragain phool jati hain | Pakhana karte waqt ya baad taaza surkh khoon; khujli ya ghanth bhi ho sakti hai | Khoon aana aam hai. Bahar wali bawaseer mein clot banne ya bawaseer bahar aane par dard ho sakta hai. Risaao aam tor par bunyadi alamat nahi hoti. |
| Anal fissure | Maqad ki andarooni jild mein chhota sa cheer | Pakhana karte waqt aur baad tez, kaatne ya jalne jaisa dard; aksar sakht pakhane ke baad | Dard aam tor par bunyadi alamat hota hai. Tissue ya pakhane par thora taaza surkh khoon aa sakta hai. Risaao aam nahi. |
| Anal fistula | Maqad/seedhi aant ke andar se qareebi jild tak ghair mamooli raasta | Ek hi jagah baar baar soojan, zakhm ya chhota suraakh jahan se risaao ho aur phir arzi tor par kam ho jaye | Peep ya risaao zyada ishara karta hai. Jab risaao ruk jaye to dard aur soojan barh sakti hai. Khoon aam tor par bunyadi alamat nahi. |
In alamat mein mushabihat ho sakti hai. Misal ke taur par ek se zyada masail mein dard aur khoon dono ho sakte hain. Sirf ghanth, khoon ya risaao dekh kar faisla nahi kiya ja sakta.
Bawaseer: aksar khoon ishara hota hai
Bahar wali bawaseer mein clot banne par dard ho sakta hai. Har maqadi khoon ko bawaseer samajhna theek nahi; doosri wajuhat bhi ho sakti hain.
Fissure: aksar dard ishara hota hai
Dard tez, kaatne ya jalne jaisa hota hai aur pakhane ke baad kuch der ya kai ghanton tak reh sakta hai. Thora taaza surkh khoon bhi aa sakta hai.
Pakhana narm rakhna aur zor na lagana naye aur halkay fissure mein arzi madad de sakta hai. Agar dard jari rahe ya baar baar ho to doctor se muaina karwana zaroori hai, kyun ke kuch fissures ko khaas ilaaj ki zaroorat hoti hai.
Fistula: aksar risaao ishara hota hai
Anal fistula ek ghair mamooli chhota raasta hota hai jo maqad ya seedhi aant ke andar ko qareeb ki jild se milata hai. Yeh infection ya phoray (abscess) ke baad ban sakta hai. Baar baar ek hi jagah soojan, peep ya pani jaisa risaao, badboo, ya maqad ke qareeb chhota suraakh is ki nishani ho sakta hai. Risaao hone par dard kuch der ke liye kam ho sakta hai, lekin masla dobara aa sakta hai.
Fistula aam tor par sirf gharelu dekhbhaal se hamesha ke liye theek nahi hota. Surgeon ka muaina aur munasib ilaaj ka mashwara zaroori hota hai.
Filhaal ghar par kya madad ho sakti hai?
Halki alamat mein, doctor se mashwara lene tak aap yeh koshish kar sakte hain:
- Agar doctor ne pani kam karne ko nahi kaha to munasib miqdaar mein pani piyen.
- Fibre dheere dheere barhayein—phalon, sabziyon, sabit anaj aur daalon se.
- Neem garam pani ka sitz bath takleef kam kar sakta hai.
- Is jagah ko narmi se saaf aur khushk rakhein; ragrein nahi.
Agar khoon bohat zyada ya lagataar aa raha ho, chakkar ya behoshi, bukhar, tezi se barhti soojan, shadeed lagataar dard, ya pakhana/peshab karne mein mushkil ho to foran medical help lein.
Ghalat fehmi vs haqeeqat
Haqeeqat: Photo poora masla bharose ke saath nahi dikha sakti. Tashkhees ke liye saamne muaina, aur kabhi kabhi mazeed tests, zaroori hote hain.
Ghalat fehmi: “Risaao band ho gaya to fistula theek ho gaya.”
Haqeeqat: Fistula kuch waqt ke liye drain ho kar chup ho sakta hai aur phir wapas aa sakta hai. Surgeon ka muaina zaroori hai.
Ghalat fehmi: “Dard ka matlab hamesha bawaseer hota hai.”
Haqeeqat: Pakhane ke saath tez dard fissure mein aam hai; shadeed soojan ya dard doosre masail mein bhi ho sakta hai.
Ahem baat
Khoon aksar bawaseer, pakhane ke saath tez dard fissure, aur baar baar risaao fistula ki taraf ishara kar sakta hai—magar yeh sirf clues hain. Khud tashkhees na karein. Asal sabab aur munasib ilaaj ke liye muaina sab se mehfooz tareeqa hai.
